How the program works

A clearer path through specialized cognitive care

Step 1

Referral + Vanguard intake

Referrals are reviewed and triaged. Many patients first move through a structured Vanguard intake process, which helps gather history, records, and clinical context before the next step.

Step 2

In-person cognitive evaluation

Patients who need specialist in-person evaluation are scheduled for a focused visit designed to clarify diagnosis, guide testing, and establish a longitudinal care plan.

Step 3

Treatment pathways

Some patients move into additional care pathways, including imaging, biomarkers, anti-amyloid therapy evaluation, and long-term monitoring when appropriate.
